To All Volunteer Staff:
At it’s 2001 meeting, the Ontario Soccer Association (OSA) Board of Directors approved a policy that all clubs must implement the Volunteer Screening Initiative Guidelines. The Volunteer Screening Initiative Guidelines document can be downloaded from Ontario Soccer Net.
The process itself is relatively simple (of no cost to you) and everything you need is included in this package. We need each Volunteer member to carefully read over all documents and complete the necessary forms (refer to the FEDS Screening Policy on which forms you need to fill out) and mail to the FEDS office 135 St Davids St South, Unit 7 Rear of Old Livery Stables, Fergus.
The Volunteer Screening package includes:
1. A photocopy of 2 pieces of Identification, (one must be a photo) must be attached to the Consent to Disclosure form. Only the following ID are accepted:
- Driver’s Licence
- Passport
- Birth Certificate
- Student Card
- Fire Arms Certificate
2. IMPORTANT: If printing the forms on-line the Consent to Disclosure form must be printed on one sheet front and back. This can be done at home/office by reloading the same page into your printer.......
